The Portfolio Document (PD) is designed to help you build a clear record of evidence that demonstrates your capabilities as a Care Coordinator. It acts both as a development record and as a practical tool to support and guide your learning in the role.

All Care Coordinators are expected to complete a PCI-accredited, two-day Care Coordination training programme, which provides the essential foundation knowledge and skills aligned with the national competency framework.

By working through this portfolio, you will be able to show how you have:

• Engaged in action-based learning to improve outcomes

• Continued your professional development beyond the initial training
